Legacy Nursing Solutions was founded by Cassandra, a seasoned nurse leader with a passion for hospice care and a heart for service. With a Master of Science in Nursing Leadership and Management degree, along with certifications as a Certified Public Health Nurse (PHN) and Certified Hospice and Palliative Nurse (CHPN), Cassandra combines deep clinical expertise with a compassionate, hands-on approach.
At Legacy, our mission is to empower hospice providers to deliver high-quality, compliant, and compassionate care. We offer a range of expert consulting services—including QAPI program management, documentation improvement, and staff education—all tailored to the unique needs of hospice agencies.

Hospice Consulting & Compliance Support

Staff Education & Training
We support hospice agencies in meeting the federally required 12 hours of annual in-service training for home health aides, as outlined in Medicare’s Conditions of Participation. Our monthly education sessions are fully customizable and built around your agency’s specific needs, real-time challenges, and current regulatory guidance. Topics range from documentation best practices and person-centered care to infection control and survey preparedness. Each session is designed to promote clinical excellence and support compliance efforts. We also provide complete, CMS-aligned training binders tailored to the topics you select or those identified during consultation—equipping your team to deliver compassionate, high-quality, and regulation-ready care.
1:1 Nurse Coaching & Mentorship
We provide personalized coaching and mentorship for new graduate nurses and those transitioning into hospice care, offering them the support and guidance needed to build confidence, strengthen clinical judgment, and develop a deep understanding of hospice philosophy and practice. Recognizing that the shift into end-of-life care can be both rewarding and challenging, our 1:1 sessions are designed to meet each nurse where they are in their professional journey. We focus on key areas such as accurate and compassionate documentation, effective symptom management, therapeutic communication with patients and families, and coping with the emotional demands of hospice work. Mentorship is tailored to address common uncertainties and build practical skills, creating a safe space for learning, reflection, and growth.
